Effect of an Intradialytic Exercise Program on Physical Function and Quality of Life of Elderly Hemodialysis Patients: A Randomized Controlled Trial

Abstract

With the global increase in elderly populations, preserving functional capacity and quality of life among older adults receiving hemodialysis is a critical healthcare goal especially those face heightened risks of falls and physical decline.
Aim of the study: Determine the effect of an intradialytic exercise program on physical function and quality of life in elderly hemodialysis patients. Method: A single-center randomized controlled trial. The study was conducted in the hemodialysis unit at Mansoura University Hospital. A purposive sample of 62 elderly patients diagnosed with end-stage renal disease was divided into two equal groups 31 elderly each. The data were collected through a Structured Interview Schedule, Six-min walk test, Second Chair Stand Test, Short Physical Performance Battery and Kidney Disease Quality of Life Short Form. Results: The exercise group showed significant improvements in physical function compared to controls. The 6-minute walk distance increased by 32.6 m (95% CI: 22.5–42.6; p = 0.009), chair stand test by 1.85 (95% CI: 1.55–2.15; p < 0.001), and Short Physical Performance Battery score by 1.25 points (95% CI: 0.99–1.51; p < 0.001). Quality of life scores improved by 10.1 points versus a decline in controls, with a large effect size (d = 2.87; 95% CI: 9.84–13.36; p < 0.001). The program was safe, with 99.2% adherence and no serious adverse events. Conclusion: Intradialytic exercise program was feasible, well tolerated, and had positive effects on physical function and quality of life in elderly hemodialysis patients. Recommendation: Incorporating such exercise programs into standard care while conducting larger trials for broader validation.
